New-client paperwork is sent by email once your child's first appointment is scheduled, and current families can find their documents in the secure patient portal.

After scheduling, a new client information form will be emailed to you. Please complete the form and return it prior to your child's evaluation and/or first therapy session. Current families can also find their documents in the patient portal.
